To participate in any sport, a student athlete must have a current sports physical form on file at school and complete the registration paperwork found on the Immaculate Conception Grade School web page. IHSA form is required by the state of Illinois for all students participating in a sport.
Please note: You may have a student with a Certificate of Health AND a Sports physical form. They are both required as they are different forms. Please plan accordingly. Failure to do so will prevent the student-athlete from participating in the specified sporting activity including practices and games.
